Background: Sarcoidosis and tuberculosis (TB) are two granulomatous inflammatory disease with some common symptoms. The purpose of this study was to compare the levels of serum biomarkers include interleukin-4 (IL-4) and IL-13, calcium (Ca), hemoglobin, sedimentation rate, and the ratio of lymphocyte-to-neutrophil among patients with pulmonary TB, patients with sarcoidosis and a control group.
Materials and Methods: This case-control study conducted in patients referred to hospital Daneshvari Still, Tehran, from April 2017 up to 2018. In this study, 24 newly diagnosed patients with active pulmonary tuberculosis, 34 patients with pulmonary sarcoidosis, and 30 healthy individuals as the control group were enrolled. The demographic data, the erythrocyte sedimentation rate (ESR), the ratio of neutrophil-to-lymphocyte (NLR), serum levels of Ca, hemoglobin (Hb), and IL-4 and IL-13 were compared between the study groups. Receiver operating characteristic (ROC) curve analysis, sensitivity, and specificity were also calculated by using SPSS 16.0 software.
Results: The mean age was 47.71 ± 10.88 and 55.25 ± 21.58 years in sarcoidosis and tuberculosis. Mean ESR in sarcoidosis patients was 21.45 ± 13.37 mm / h and 41.4 ± 17 mm / h in the TB group. The percentage of peripheral blood lymphocytes in patients with sarcoidosis and tuberculosis was 28.02 ± 12.20 and 21.41 ± 12.49, respectively, which were significantly higher among patients with sarcoidosis. NLR also 2.4 ± 1.6 and 4.4 ± 2.9 in patients with sarcoidosis and TB, respectively, which showed significant differences between the groups. Regarding the evaluation of the level of IL-4 and IL-13 in patients, it should be noted that IL-4 in patients with sarcoidosis was 90 pg / ml compared to 20 pg / ml for TB patients (P <0.001).
There is no significant difference in the level of IL-13 in the TB and the control group, which varies between 20 and 80 pg / ml (P = 0.35). However, its value was significantly higher in patients with sarcoidosis (P = 0.01) compared to the healthy control group and tuberculosis (P = 0.01). ROC curves showed that the diagnostic cutoff level of ESR, Ca, NLR, and Hb can be valuable because the area under the curve. The TLC cutpoint of 34 mm / hour to ESR had 86% sensitivity and 80% specificity to distinguish TB from the sarcoidosis.
Conclusion: The serum levels of the biomarker showed a strong background in immunology of sarcoidosis using NLR, Ca, ESR, and Hb.

Novel role of interleukin 13 alpha2 receptor in perineural Invasion and association with Poor Prognosis of Patients with Pancreatic Ductal Adenocarcinoma
Perineural invasion (PNI) is one of the main pathologic characteristics of pancreatic ductal adeno-carcinoma (PDAC), which is mediated by attacking cancer cells into nerve cells. Here, we identify the overexpression of interleukin-13 receptor alpha2 (IL-13Rα2) in PNI of 236 samples of PDAC by studying the expression on protein level by immunohistochemistry (IHC) and the level of RNA by in situ hybridization (ISH). We observe that ≥75% of the sample expressed IL-13Rα2 by IHC and ISH in grade 2 and 3 tumors, whereas ≥64% stage II and III tumors expressed IL-13Rα2 (≥2 +).
Interestingly, ≥36% peripancreatic nerve plexus (PL) and ≥70% nerve ends (Ne) between PNI in PDAC samples showed higher levels of IL-13Rα2 (≥2 +). IL-13Rα2 PL and Ne ve subjects survived significantly less than IL-13Rα2 ve subjects, suggesting that IL-13Rα2 may have a unique role as a biomarker of PNI-aggressiveness.
